Gout Flare Hot Weather: Why Heat Triggers Attacks and How to Avoid Them
Gout flare hot weather is a real concern in Malaysia, especially during heat alerts, long outdoor days and festive meals. Gout is a painful form of inflammatory arthritis caused by uric acid crystals in a joint. It often starts as sudden gout big toe pain, but it can also affect the ankle, midfoot, knee, wrist, elbow or fingers.
As an orthopaedic surgeon, I often see patients who think they have a sprain, infection, tendon problem or fracture, when the real cause is a gout attack. The joint can become red, swollen, hot and so painful that even a bedsheet feels too heavy.
Hot weather does not cause gout by itself. However, heat can make existing high uric acid more likely to turn into a flare. Dehydration, sweating, sugary drinks, alcohol and purine-rich meals can all add to the risk.
Why Hot Weather Can Trigger a Gout Attack
Research has found a link between higher temperatures and recurrent gout attacks. In one case-crossover study, higher temperatures in the previous 48 hours were linked with about a 40% higher risk of a gout attack compared with moderate temperatures. The highest risk was seen when high temperature was combined with low humidity.
This does not mean every hot day will cause gout. It means that heat can be one more trigger in someone who already has uric acid crystals in the body.
The most likely reason is fluid loss. In hot weather, the body sweats more. If you do not replace that fluid, the blood and urine become more concentrated. As a result, uric acid may rise and the kidneys may clear it less well. In a person with gout, this can spark uric acid joint pain.
There is another reason the big toe is commonly affected. The big toe joint is far from the centre of the body and is exposed to pressure from walking and footwear. Therefore, uric acid crystals often settle there, causing classic gout big toe pain.

Dehydration and Gout in Malaysian Heat
Malaysia’s hot weather alerts make this topic especially relevant. MetMalaysia uses a Level 1 hot weather status when daily maximum temperatures reach 35 to 37°C for at least three consecutive days, while Level 2 heatwave conditions are higher again. During these periods, people who work outdoors, exercise, fast, travel, or spend long hours in non-air-conditioned places can lose a lot of fluid.
Dehydration and gout are closely linked because the kidneys help remove uric acid. When fluid intake is low, uric acid may build up more easily. Signs that you may not be drinking enough include dark urine, passing urine less often, dry mouth, headache, dizziness and feeling unusually tired.
However, hydration advice is not the same for everyone. If you have heart failure, kidney disease, severe liver disease, or you have been told to restrict fluids, follow your doctor’s plan. Do not suddenly force large amounts of water without medical advice.
Common Gout Triggers During Hot Weather
Heat is often only one part of the story. Many gout attacks happen after several gout triggers appear together.

Festive Food, Seafood and Gout Diet Malaysia
In Malaysia, gout often flares after festive meals, weddings, open houses, seafood dinners or late-night makan sessions. This does not mean you can never enjoy local food. It means portions and patterns matter.
Foods and drinks that may trigger gout include large portions of red meat, organ meats, rich meat gravies, shellfish, prawns, anchovies, sardines, beer, spirits and sweet drinks. In addition, eating a lot more than usual over a few days can be enough to trigger symptoms in someone with high uric acid.
A practical gout diet Malaysia approach is to keep the plate balanced. Choose water. Keep meat and seafood portions modest. Add vegetables. Avoid repeated heavy meals on consecutive days. Low-fat dairy may be useful for some people. Most vegetables, including those with moderate purine content, are not usually the main problem for gout.

How to Prevent Gout Attack During Hot Weather
If you are prone to gout, prevention works best before the joint starts hurting. These steps can help lower gout flare hot weather risk.
- Drink water regularly, especially if you sweat, walk outdoors, exercise or work in the heat.
- Use urine colour as a simple guide. Very dark urine often means you need more fluid, unless your doctor has restricted fluids.
- Limit alcohol during hot spells, especially beer and spirits.
- Avoid using sweet drinks as your main “rehydration” drink.
- Keep festive meals moderate. Avoid combining a large seafood meal, alcohol and dehydration on the same day.
- Wear comfortable shoes with enough space around the big toe.
- Build exercise gradually. Sudden long walks, hikes or sports after a break can stress the foot and ankle.
- Do not stop prescribed uric acid medicine just because you feel well.
- Keep your follow-up blood tests, especially if you are on urate-lowering treatment.
For people who fast, prevention needs planning. Hydrate well during non-fasting hours if your doctor allows it. Avoid a very salty or very sweet meal at sahur. During buka puasa, break the fast gently and avoid making a large purine-rich meal the first heavy meal after a long hot day.
What to Do if Gout Big Toe Pain Starts
A gout attack usually builds quickly. The joint may become intensely painful within hours. Early treatment often works better than waiting several days.
Basic steps include resting the joint, raising the foot, using a cold pack wrapped in a towel, and avoiding tight shoes. Do not place ice directly on the skin.
Medical gout treatment may include anti-inflammatory medicine such as colchicine, a non-steroidal anti-inflammatory drug, or a corticosteroid. The best choice depends on your kidneys, stomach, heart, diabetes status, other medicines and past side effects. Therefore, do not borrow another person’s gout medicine.
If you already take allopurinol or febuxostat, do not stop it suddenly during a flare unless your doctor tells you to. Stopping can cause uric acid levels to swing and may make future gout control harder.
When Uric Acid Joint Pain Needs a Longer Plan
Hydration and diet can reduce gout triggers, but they may not be enough if uric acid remains high. Repeated gout attacks mean the body may already have a significant crystal burden. Over time, gout can damage joints, tendons and bone, and it can form lumps called tophi.
Long-term gout treatment often aims to lower serum uric acid to a target level. Many guidelines use a target below 6 mg/dL, or below 360 micromol/L. A lower target may be used if tophi are present. This usually needs regular blood tests and dose adjustment, not guesswork.
An orthopaedic surgeon can help assess a painful swollen joint, check for joint damage, and rule out other causes such as infection, fracture, cartilage injury or tendon disease. Long-term uric acid control is often shared with a physician, rheumatologist or primary care doctor.
When to Seek Urgent Care
Do not assume every red painful joint is gout. Seek urgent medical care if:
- This is your first ever attack of severe joint swelling.
- You have fever, chills or feel very unwell.
- The joint is extremely red, hot and swollen.
- You cannot bear weight or cannot move the joint.
- The pain started after a fall, twist or injury.
- You have diabetes, poor immunity, kidney disease, a wound near the joint, or a recent procedure.
- The pain is not improving with proper treatment.
Joint infection can look like gout and can be dangerous. A doctor may need blood tests, X-rays, ultrasound, or joint fluid testing to confirm the diagnosis.
Conclusion
Gout flare hot weather risk rises because heat, sweating and dehydration can push uric acid levels in the wrong direction. In Malaysia, this risk often combines with festive meals, seafood, alcohol, sweet drinks, outdoor work and long travel days.
The best prevention is practical. Keep hydrated if it is safe for you, avoid stacking several gout triggers on the same day, take prescribed gout treatment correctly, and seek medical care early when a flare starts.
