When Young Knees Fail: Navigating ACL & Meniscal Tears in Teen Athletes

Every pivot, jump, and sprint carries a higher price for the adolescent knee. ACL injuries in teens and meniscal tears are becoming more common, especially among active youth. As an orthopaedic surgeon, I often see these injuries in teenage athletes. Understanding these injuries can help us better protect the next generation.

Anatomy & Growth Plate Considerations

Adolescent knees are different from adult knees. Their bones are still growing, making them more susceptible to injury. The growth plates near the knee joints add complexity. Damage to these areas can impact future growth and knee function.

Signs & Symptoms to Watch For

The first sign of an ACL injury is often a “pop” sound, followed by immediate swelling and pain. Meniscal tears typically occur alongside ACL ruptures, causing further complications if not addressed quickly. Watch for swelling, instability, and difficulty bending or straightening the knee.

Diagnostic Advances

Early and accurate diagnosis is critical. MRI scans are highly effective in identifying ACL injuries and meniscal tears.

Treatment Pathways

Treatment depends on the injury severity and the athlete’s age. Conservative rehabilitation focuses on strength and stability without surgery. For serious injuries, ACL reconstruction or meniscus repair might be necessary. ACL reconstruction involves rebuilding the ligament, while meniscus repair aims to preserve knee cartilage.

Prevention Strategies

Prevention is always better than cure. Neuromuscular training programmes, focusing on strength, agility, and proper landing techniques, significantly reduce injury risk. Consistent and supervised training can prevent many knee injuries in adolescent athletes.

Rehabilitation & Return to Play

Rehabilitation after surgery or injury is crucial. Athletes usually take 6 to 12 months to return safely to their sport. The return-to-play decision should be based on strength, balance, and functional tests rather than timelines alone.
